• THIS FORM IS ONLY FOR ROLLER/INLINE SPEED PARTICIPATION.
• To Register for this tournament, player has to enroll for RSFI registration,if already enrolled then can fill the form as below or else go for RSFI registration first on this link as given and then there after fill the form for this championship below.

• Only professional skate are allowed .No toy skates are allowed.
• Last Date to fill this tournament online form is 21st October 2025 upto 9 :00 PM.
• All the participant have to participate in two races.
• For participants of age group above 10 yrs. Are having one rink race & one road race .(Race chart schedule enclosed )
• For participants of age group below 10 yrs have both Rink races ( Race chart schedule enclosed).
• For participants below 6 yrs, this event is for promotion purpose upto State level only if eligible and shall not be allowed further for National championships.
• NO FORM WILL BE ENTERTAINED THEREAFTER UNDER ANY CIRCUMSTANCES.
• No corrections will be entertained once the form is submitted. Therefore, we request you to please fill in your details carefully before submitting the form. Also, please make sure you enter the correct WhatsApp mobile number so that further communication can take place.
• Entry fee once submitted will not be refunded or adjusted in any circumstances/manner whatsoever.
• Dates, Schedules & Venues are subject to change at any point of time on or before the conduction of this Championship .
• For those Participants having Road race will have to report for their race on 26-10-2025 at 6:00 AM. The venue will be shared later via WhatsApp.
• Online submission of the form by the participant/parents implies the consent of the participant/parents to agree to all the terms and conditions in any manner whatsoever as given in this form.
• Once you have submitted the form online, please ensure to submit the following documents IN TWO SETS as mentioned below in Hard Copy/Print Out: A copy of your Downloaded Entry form (2025), A copy of your Aadhar card or School bonafide copy (Mentioning Participant's Full Date of Birth), An entry fee of Rs500/- .
• All the above documents are to be submitted via your coach .If in case ,you have no coach /club .then fill NA in the respective column and such entry form can be accepted by the participant directly.
• Chest no will be issued to your coach at the time of Submission of above documents & Entry Fee.
• The Entry form in hard copy with all above documents as mentioned above (In Two sets) has to be submitted at ROLLER SKATING ASSOCIATION FARIDABAD with Mr. ASHOK KUMAR SHARMA by 24th Oct 2025 (Mobile no: 9810201810) and collect their chest no.
